The reason its possible is because Second Life is a sandbox game, and all of the adult content and child avatars are just user-created content within that sandbox.
They could require all adult content and all child avatars to be registered to some system that would make them not possible to use together, but if people are going to break the rules anyway then they would be just as happy to break a rule that says "child avatars must be registered as child avatars to prevent them from being used with adult content". Keep in mind that its possible to be a "child avatar" without using any specific child-shaped mesh body.
There's also a huge amount of conduct that is forbidden under the sexual ageplay rule that no automated system could feasibly cover, or would be extremely disruptive to players.
